If you look after someone who needs your help because of their illness, frailty, disability or addiction then you are a carer. You can ask the person you care for to complete a ‘consent to share’ form if they want you to be able to discuss their health details.
You can register as a carer without the knowledge of the person you care for and they do not have to be a patient at the surgery.
If you are a carer or are cared for by a voluntary carer please let us know, we can put carers in touch with local support groups.
If we know which of our patients has a carer or is a carer then this will allow the Practice to care for you fully, by being aware of the needs of both the patient and the carer.
