If you are already registered at this practice it is important that you keep us informed as soon as possible of any changes to your name, address, telephone number/s and email address.
This practice has a policy to check the identification and residency of all patients. This applies to new registrations and patients already registered who may have had a change to their details as above. For a change of details to be recorded you will need to produce documents such as a marriage certificate, change of name Deed Poll documents or proof of new address. This is recommended by the Department of Health and is designed to reduce NHS fraud.
