Lloyds Pharmacy and Fordingbridge Pharmacy, in Fordingbridge, operate a collection service from the Surgery. Please mark your repeat slip clearly if you want to use this service.
Non-Dispensing Patients on Repeat Medication
Please remember you can pick up your paper prescription from the Dispensary here at the Surgery or you can request your prescription is sent to either of the chemists in Fordingbridge (Lloyds Pharmacy or the Fordingbridge Pharmacy). It is VERY IMPORTANT that you clearly indicate on your repeat medication slip this information so the Dispensary team are aware of your wishes.
If an ink stamp is showing on your repeat slip, please check this is the correct chemist you are requesting your prescription is sent to.
We may ask you or your representative to sign for the receipt of prescriptions or medication for certain "controlled" drugs. We may ask your representative for written permission to give them your prescription or medication. We will ask for proof of exemption from prescription charges from our dispensing patients. We do not normally give prescriptions or medication out to children under the age of 16 unless they have a signed request from the patient.
