Long Term Conditions Practice Nurse

Full Time or Part Time

An exciting opportunity has arisen for a Practice Nurse with a special interest in Long Term Conditions (Chronic Disease) to join our friendly, progressive GP training practice on the edge of the beautiful New Forest in Hampshire.

We are looking for an experienced, flexible and highly motivated individual to join our team. Experience of general practice and chronic disease management of respiratory conditions are essential.

We provide a high standard of patient-centered care and the successful candidate will become a valued member of our practice team and supported through further training and development.

We offer a competitive, negotiable salary dependent on experience, NHS Pension, five weeks annual leave, plus bank holidays.
